• Collaboration & Networking: Work closely with relevant organizations to ensure comprehensive care for the elderly, fostering collaboration in patient care.
• Support for Destitute Patients: Address the social needs of patients who are destitute, ensuring they receive the necessary care and support.
• Multi-Disciplinary Team Participation: Contribute to the patient-care team, ensuring social work perspectives are integrated into treatment planning and care delivery.
• Groupwork: Conduct groupwork with elderly patients or their caregivers, offering support and resources to improve their well-being.
• Support Programmes: Lead initiatives and implement support programs aimed at enhancing the care and support of elderly patients and their caregivers within the community.

Job Requirements:

• Degree and postgraduate diploma in Social Work.
• Must have an interest to work with the elderly in a healthcare setting
