The Community Nurse (Home Care) is responsible for planning; coordinating, directing and evaluating the patient care services and provides supervision serving as the vital link between the vision of CWA and a team of Community Caregivers (CCGs). It served as the unit-based delivery of effective, cost-efficient high quality patient care. It also includes providing quality care to clients under the Home Monitoring System.

The Community Nurse (Home Care) is also responsible for managing and providing direct patient care through care management, assessment, providing home personal care services, nursing procedures and evaluation of nursing care to meet the needs of the patients, including educating patients & families and caregiver training.

DUTIES AND RESPONSIBILITIES

1) Home Personal Care (HPC)

a) Patient Care

· Assess, plan, implement and evaluate patients’ needs

· Provide management and supervision to a team of Community Caregivers (CCGs) and in-house training of CCGs

· Facilitate patient flow and patient education

· Refer patients for further care or services, when necessary

HR Generalist
FULL TIME: $2800 - 3200

8am - 5pm (5.5 Working Days)

Roles & Responsibilities:

· Responsible in doing full spectrum of HR duties including payroll.

· Assist in HR matters in manpower planning, recruitment, compensation, and performance management

· Administer foreign worker work passes related matter, i.e. application, renewal and cancellation

· Handle employees’ onboarding / orientation and off-boarding processes

· Handle employees' leave, attendance, welfare, and benefits

· Handle submission of government grants and claims

· Administer, maintain and update HR policies and procedures

· Administer employees’ competency and appraisal matrix

· Handle employee disciplinary issues and grievances

· General administrative duties (data entry, filing, answering incoming calls, license renewal etc)

· Assists in corporate events and staff recreational activities

· Arranging and scheduling meetings, setting up video and/or teleconferences and preparation of meeting packs

· Any other relevant duties as assigned

Job Requirements:

· 5.5 Working day

• Proficient in Microsoft office (Outlook, Word, Excel )

· Computer literate and handle emails / correspondences

· GCE ‘A’ Level or Diploma in HR/Business administration or related field

· Minimum 2 years relevant working experience in Human Resources Functions

· Excellent team player and highly dependable

• Knowledge of human resources policies, MOM rules and regulations
• Ability to maintain confidentiality in HR matters
• Ability to interact and work with staffs of all levels (need to interact with foreign workers)
• Pro-active attitude, responsive, able to multitask well & eye for details
• Ability to exercise initiative, work independently and under pressure
• Strong organizational skills
• Immediate availability or can start work within short notice will be of great advantage
