Proficient in English and Chinese (oral and written)

Strong communication, people skills and Problem-solving skills

Good organizational and multi-tasking abilities

Able to perform Basic Grooming skills is a plus point

5 Day Work Week (have to work on weekends & some PH)

Relevant experience will be advantageous, otherwise we can train you if you have a willing to learn attitude

Location: Punggol

Job Responsibilities:

• Conduct group activities for carers and/or persons with dementia.
• Engage with various stakeholders to ensure the smooth running of programme.
• Support outreach events and initiatives.
• Explore new avenues of collaboration with community partners and stakeholders.
• Provide case management support to carers of persons with dementia.
• Ensure proper documentation of client follow-ups.
• Update statistical data in a timely manner for funder reporting.
• Perform other duties as assigned.

Requirements

• Degree in Psychology, Social Work, Community Development, or a relevant discipline, with a minimum of 2 years of experience in community engagement and/or case management.
• Prior experience working with persons with dementia or caregivers is an added advantage.
• Ability to work on Saturdays.
• Self-motivated with a high level of initiative.
• Ability to plan and implement ideas effectively.
