The Staff Nurses collaborates closely with the doctor and social worker to provide home-based nursing service to the frail and home bound elders to meet the our companys mission and operational targets, in accordance with the standards of care and service protocols established by the Foundation and the community.

Key Responsibilities

• Conduct home visits and perform comprehensive nursing assessment regularly to homebound frail elders
• Able to triage client condition and respond in a timely manner to address the issue
• Be part of the interdisciplinary team to develop and implement client’s care plan
• Explain care goals and care plans to client and caregivers to gain active participation from them

Job Description

• Provides home personal care services such as assistance with activities of living (eg. showering, dressing, feeding, toileting, transferring, moving around), eldersitting, medication, mind-stimulating exercises, and simple exercises.
• Works closely with the clinical team to ensure that the patient is well taken care of.
• Assists nurse-in-charge to perform biopsychosocial and environmental assessment on the initial visit.

1. Programme Planning and Implementation

• Oversee and manage the Alert Alarm System (AAS) during the centre’s operating hours, rendering assistance when necessary.
• Establish a system to manage the AAS outside operating hours and ensures that help continues to be available when needed, including a system of recalling staff outside operating hours.

3. Volunteer Development

• Establish networks with local community-based services/ resources, such as grassroots, social services, corporate organisations and schools, to engage volunteers to help run or assist in the programmes, start income-generating activities, conduct outreach efforts etc., to match the needs of seniors.
• Recruit, interview and screen potential volunteers.
• Provide and coordinate orientation of volunteers to their assigned role and the mission and objectives of the centre.

• Establish a system of recognition for the volunteers.

Key Responsibilities

• Provide daily escort duties for clients from their home to the hospital or polyclinic for their medical appointments.
• Accompany the clients on the vehicle and keep an eye on them during the journey.
• Assist client with walking aids or on wheelchairs with their ambulation.
• Assist the clients throughout their hospital or polyclinic visit, while ensuring their safety at all times.
• Escorts/transports clients safely, professionally, courteously and in a timely manner throughout the hospital or polyclinic.
• Assist clients in boarding and alighting from the vehicle in line with clients’ needs; where necessary, via operation of wheelchair hydraulic lift or other mobility devices in line with proper, safe lifting techniques.
• Administer safety procedures for clients before and during transportation to ensure safe and secure vehicle environment. Ensure that clients/wheelchairs are safely secured in the vehicle.
• Escort the right senior to the right medical institution/clinic and room(s) for their medical treatment/consultation.
• Perform with medical appointment process such as clinic registration, settling of payments, future appointments scheduling, walk-in and collection of medication.
• Promote safety and comfort for seniors by interacting with them while waiting for transport during appointments.
• Liaise with office with client’s follow up medical appointment and transportation.
• Attend to senior’s needs and provide physical assistance such as buying meals, toileting, perform translation if needed) during appointments (i.e. on wheelchair)
• Hand over all medications, doctor’s notes, appointment schedules to the elderly or cluster support staff or social worker upon return to the Home.
• Inform the supervisor/manager of any abnormality of the client.
• Physically able to handle elderly on a wheelchair.
