Purpose of job

Provide leadership and oversee service delivery, training for volunteers according to programme objective in accordance to ethical and legal guidelines.

Key Responsibilities Areas

• Work closely with the Team Leads to identify potential clients who are in recovery according to programme requirements.
• Work with Crest Team Lead to develop strategy to operationalise plans in volunteer recruitment, design of training materials, befriender training, budget planning, execution and evaluation of programme.
• Provide care coordination: Develop and monitor care plans, referral and follow-up for clients and caregivers who are at-risk of mental health and dementia.
• Plan and conduct outreach and screening to the community, including community mapping, to promote mental health awareness and early identification of mental illnesses and dementia.
• Plan and organise cognitive stimulation activities or programmes for the community to support clients who are at-risk of cognitive decline, mental health conditions and dementia.
• Coordinate and match potential volunteers to clients to maintain social connectedness through befriending the clients.
• Plan and conduct training for volunteers in their roles and responsibilities to carry out befriending, and identifying red flags and mental health symptoms.
• Networking with relevant stakeholders to promote the interests and services of Filos Community Services.
• Documentation of visits and sessions conducted by volunteers to clients to meet the programme KPI.
• Provide monthly administrative reporting according to service requirement, and manage the data base of clients and volunteers.
• To undertake duties assigned by the Executive Director, Head and Team Leads.

Requirements

Experience:

• Knowledge in programme implementation, volunteer management and training.
• Experience in case management, groupwork and community work will be an advantage.
• Experience and/or knowledge in working with clients on Mental Health challenges and Person-Centred Care and recovery will be an advantage.
• Good organisational skills, highly motivated and service mindset to serve the community.
• Ability to work independently and as a strong team player.
• Good administrative and computer skills with proficiency in MS Office.

Job Description:

• Responsible for tasked BIM Modelling works and shop drawings required by the projects and relevant submissions.
• Prepare drawings & BIM Modelling accurately according to the instructions/directions from the PM/PD for construction or building works.
• Ensure that BIM models are delivered in quality, due diligence and timely manner.
• Identify any discrepancies occurred for BIM models during the design stage.

1. The Construction Manager shall play a key role in the conceptual development of the various construction projects and oversee its organization, scheduling, and implementation
2. He/She will be required to plan, lead and monitor progress and performance against the project plan whilst managing the day-to-day operational aspects of projects which includes manpower planning, construction progress monitoring, preparation of master schedule and cost control

Requirements:

RESPONSIBILITIES

Our Medical Social Workers collaborate with a multi-disciplinary team of stakeholders to regularly review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. They will be involved in the means-testing review, and provide fair payment proposals, financial application assistance, and any appropriate assistance based on the social & economic status of the family and/or resident.

Admissions

• Provide financial counseling upon admission

Case Conference

• Review social aspects inward with regards to activity participation, visitors and befriending, financial consideration status, etc.
