Residents’ Case Management (Adults & Children)
1. Planning, coordinating, and implementing support packages:
- Develop and manage comprehensive support plans to assist clients and their families in overcoming challenges and dependencies.
2. Client interviews and assessments:
- Conduct thorough interviews with clients to assess their current condition, needs, strengths, and challenges.
3. Tailored goal setting:
- Treat each case as unique, setting specific, measurable goals tailored to the client's situation.
4. Crisis model case management:
- Provide case management services using the crisis model approach to address immediate needs and long-term stability.
5. Monitoring and evaluation:
- Continuously monitor, evaluate, and record the client's progress against the set measurable goals.
6. Referral to related agencies:
- Connect clients with other social and professional services to ensure comprehensive support.
7. Family cohesion in the shelter:
- Facilitate activities and initiatives to build and maintain cohesive family units within the shelter environment.
8. Resident status and financial assistance recommendations:
- Assess and recommend extensions of resident status and financial assistance, highlighting any pertinent issues.
9. Collaboration with school counselors:
- Work with school counselors to support school-going children, monitoring their behavior and academic progress throughout the year.
10. Case conferences coordination:
- Organize and coordinate case conferences with referring agencies such as Family Service Centres (FSC).
11. Accompanying residents to official meetings:
- If necessary, accompany residents to official meetings with related agencies (e.g., Syariah Court, Housing Development Board, hospitals).
12. Facilitation of discharge plans:
- Develop and implement discharge plans with clients, ensuring follow-up and post-care support.
Empowerment Programmes (Adults & Children)
1. Program coordination:
- Organize and oversee empowerment programs designed for clients, ensuring they meet the intended objectives.
2. Service provider coordination:
- Identify and coordinate with service providers to deliver program components effectively.
Administrative Duties
1. Documentation and filing:
- Ensure all case notes are accurately documented and properly filed.
2. Monthly reports preparation:
- Compile and prepare monthly reports detailing the progress and status of clients and programs.
Additional Duties
- Perform any other tasks as assigned by the immediate supervisor.
