Job Scope:

• Responsible for the triage for specialist outpatient clinic appointment requests received from different referral steams
• Consolidate appointments for patients where appropriate
• Assist to confirm patients’ attendance especially for first visit appointments, and assist in cancelations, rescheduling or ad hoc patient enquiry
• Contribute to the implementation of department plans to right site outpatient referrals via risk stratification on the basis of available historical clinical, biochemical and radiological information
• Work closely with referring healthcare stakeholders regarding the outcome of the outpatient referral request made
• Assis to plan outpatient visit as per available clinic slot based on estimated urgency to be seen
• Collate and evaluate appointment waiting time data for department review
• Responsible for proper documentation of appointment allocation and triaging data, with adherence to PDPA policies

Job Responsibilities

• Oversee the daily operations of the student care centre efficiently and effectively.
• In charge of curriculum professional matters and oversee to ensure that programmes and activities are carried out as centre’s curriculum

Job Summary

The Care Advisor (2-year contract) supports the Manager (Team Lead) to spearhead Yayasan MENDAKI’s push towards last-mile service delivery (LMSD) to better meet the needs of the Malay/Muslim community. Yayasan MENDAKI is embarking on a strategic client-centric initiative to achieve the next bound of service delivery, by proactively reaching out to families to understand their needs, care for them and partner with them in their journey of lifelong learning.

Key Responsibility Areas

Outreach - Implement new outreach methods and channels within the boundary of the

satellite centre.

• Work with the team and team lead to reach out to different segments of the community, including identifying and establishing contact with under-served segments
• Support the planning, deployment and assessment of outreach efforts

Care - Experiment with innovative client-centric approaches.

• Work with the team and team lead to pilot different ways to assess the needs of clients, including testing and refining them over time.
• Recommend suitable interventions to meet the needs of clients, which may not be limited to Yayasan MENDAKI’s programmes or national programmes, but all other solutions, such as enlisting the help of volunteers.
• Think outside the box when partnering with the client in problem-solving to meet his/her needs.
• Connect clients to resources and facilitate them in tapping on these resources.
• Advocate for clients at both the individual and macro level, based on ground sensing of the service delivery gaps through our engagements.

Stakeholder Management - Develop good working relationships with local stakeholders under his/her jurisdiction. This includes working with the M3 partners of MUIS and MESRA as well as government and social service agencies in the neighbourhood under his/her jurisdiction.

• Maintain a repository of resources available to meet clients’ needs.
• Develop an effective and efficient process to collaborate with stakeholders.

Administrative - Work with the team in the smooth running of satellite centre operations.

• Collate satellite centre activity data, including activities run in the vicinity or M3@Town, to provide progress updates to management
• Monitor the use and maintenance of satellite centre space and equipment;
• Collaborate with internal departments and partners for utilisation of satellite centre or partner’s premises

In achieving the above, the Care Advisor will work with the team under the direct and close guidance of the Team Lead and with the support of the relevant support departments.
