RESPONSIBILITIES
The Programme Officer contributes actively to the programme needs of MINDS Future Care Planning (FCP) Services under the umbrella of Community-Based Support Services (CSS). CSS is the dynamic and nimble service delivery arm in MINDS that responds to emerging needs of PWIDs and Caregivers in the community. The programme executive will work in synergy with various direct community services to enhance integration and community living of caregivers and PWIDs in the community.
He/she will be involved in the planning, coordinating and executing of programme events and activities that suit the needs of families of persons with intellectual and developmental needs. He/she will contribute actively to FCP Services and work with team to ensure the efficient running of FCP programmes which will include working with caregivers on planning for the future, life coaching and life planning. He/she will also assist the team in developing and enhancing MINDS online future planning website (FutureReady by MINDS).
A proactive thinker, resourceful and responsible team player, he/she works closely with team to raise awareness of future care planning matters, plan programmes and ensure smooth delivery of programme. He/she will also connect and coordinate with other organisations to execute collaborative, inclusive and client-centric programmes, to manage communications and feedback from stakeholders, and to conduct outreach activities to other organisations and the community.
Information/ intake assessment and life plan development
- To attend to queries and referrals on FCP programme
- To triage and refer client to suitable programmes and services
- To arrange for assessment with clients and caregivers to explore and provide guidance on future planning
- To support the social worker in drawing up individual life plans for client and work with life coaches and caregivers to set goals and plans to achieve the set goals.
- To periodically review the life plan with client and caregiver
Build Stakeholder Relationship
- Manage communications and feedback from caregivers
- Coordinate with internal and external agencies for the execution of events and/or activities
- Develop and support outreach activities to client group/s, other organisations and the community
Programme Management and Administrative Support
- Supports the Manager in coordinating and executing assigned events and/or activities.
- Content creation and/or organising of publicity materials for distribution to key stakeholders
- Support the Manger in the collation of content resources and information and assist in the enhancement of FutureReady toolkit
- Maintain organised records on progress of assigned events and/or activities
- Support and conduct needs assessment with key stakeholders
- Supports the Manager in clerical, administrative, financial and procurement matters
QUALIFICATIONS
- Diploma holder in any disciple
- Certificate in basic case management will be an advantage
OTHER INFORMATION
- Relevant Digital literacy and mass communications certificate
- Social Service sector / programme management experience will be an advantage
- Hands on with strong communication and good interpersonal skills with an ability to influence
