As the Head of Community Care at Loving Heart, you will lead and oversee the strategic direction, programme development, and service delivery for community care initiatives.

This includes driving comprehensive case monitoring, building and managing stakeholder relationships to create a strong referral network, and developing impactful programmes for vulnerable individuals. Reporting to senior leadership, you will play a pivotal role in aligning services with Loving Heart’s mission and community care goals.

Responsibilities:

1. Strategic Case Monitoring

• Lead and refine case monitoring processes to ensure vulnerable individuals receive holistic, tailored support based on their unique needs.
• Oversee the development and review of personalised care plans, coordinating interventions that address social, health, and lifestyle needs.

2. Stakeholder Engagement & Partnership Development

• Build and sustain productive relationships with key stakeholders, including healthcare providers, social services, and community partners, to create a comprehensive referral network.
• Engage with stakeholders to streamline service delivery and increase support for vulnerable community members.
• Act as a key representative of Loving Heart in stakeholder meetings and community outreach efforts, enhancing visibility and strengthening partnerships.

3. Programme Development & Implementation

• Direct the design, implementation, and evaluation of programmes to meet the needs of vulnerable community members, ensuring alignment with the organisation’s values and strategic goals.
• Guide community outreach initiatives and identify new programme opportunities based on evolving community needs.
• Oversee needs assessments to ensure programme relevance and impact.

4. Leadership & Team Development

• Lead and mentor the Community Care team, fostering a culture of learning and growth that builds capacity and enhances team skills.
• Ensure comprehensive documentation and reporting on programme activities, case monitoring, and outcomes to meet internal and funder requirements.

5. Other Duties

• Perform any other responsibilities as assigned by reporting officer.

Qualifications & Requirements:

• Advanced qualification in Social Work, Community Care, Gerontology, or a related field
• 4+ years of experience in community care, social services, or a related sector, with a focus on vulnerable populations
• Demonstrated experience in partnership and stakeholder management
• Strong programme development and evaluation skills
• Excellent communication, leadership, and interpersonal skills
• Proficiency in data management and Microsoft Office tools

Reporting to the Centre Supervisor, you will work collaboratively to support the daily operations of the assigned Active Ageing Centre (AAC), ensuring its activities and services align with the objectives set by funders and Loving Heart (LH).

Key Responsibilities

1. Programme Facilitation

• Conduct active ageing programmes within and beyond the centre to engage seniors.
• Provide befriending and buddying services to frail and vulnerable elderly in their homes.

2. Social Support & Community Engagement

• Serve as a social connector, supporting seniors in adhering to health plan recommendations through social and lifestyle interventions.
• Conduct community outreach and assess the needs of elderly residents within the designated service area.
• Make referrals to appropriate services for elderly individuals in need of further assistance.

3. Administrative & Reporting Duties

• Enrol new members, maintain accurate attendance records, visitation notes, and compile data or statistics required by funders.
• Manage invoicing, fee collection, cash handling, and banking.
• Oversee documentation, including report submissions, claims, and filing. Assist with audit preparations and documentation.

4. Centre Operations

• Perform various tasks to ensure the smooth operation of the centre.
• Undertake additional responsibilities as directed by the reporting officer.

Qualifications & Requirements

• Relevant qualification in eldercare or social services
• At least 1 year of experience in direct operational roles, especially in eldercare, including programme facilitation and needs assessment
• Experience in stakeholder management, particularly with community partners
• Proficiency in Microsoft Office
• Strong administrative and problem-solving skills
• Excellent interpersonal and communication skills; team-oriented yet capable of working independently
• Enjoys engaging with seniors and volunteers
• Bilingual abilities with proficiency in dialects preferred

Main purpose of the job

To perform repair, inspect and commission Company’s System (cranes, drilling systems, pipe-lay systems etc.) at the client’s premises or at the Company under supervision of an experienced service engineer

Responsibilities:

• Support the Senior/ Service & Commissioning Engineer on site.
• Provide technical expertise and carryout service order assignments from the Service & Commissioning Manager/ Service & Commissioning Coordinator
• Field representative of the company to evaluate, troubleshoot, analyze, repair, test, and return equipment to service as required or instructed by a service order in a safe and effective manner.
• Create daily reports about the performed service tasks and all related issues and send them to the Service Mailbox
• Complete all the required documentation & write reports regarding the work

Requirements:

• Minimum a Degree in Electrical, Electronic, Computer Science or equivalent
• Good, basic, technical insight into electrical, hydraulic and basic mechanical engineering fundamentals
• 1 year of relevant experience as Service Engineer in Oil & Gas/Marine industry

• Basic understanding of programming and industrial control systems is a plus
