RESPONSIBILITIES
The Staff Nurse is responsible for facilitating change and improving lives for individuals, families, community facing transitional challenges through the provision of direct services.
He / She will act professionally at all times and be accountable to the client and the organization to ensure that intervention plans are developed and implemented according to professional judgment and knowledge.
Job Responsibilites
- Render care to clients in accordance to Singapore Nursing Board (SNB) ‘Code for Nurses and Midwives'
- Assess and accept clients to programme according to eligibility and complete all necessary assessments and documentations
- Develop Individualised care plan base on client's needs with person-centred approach
- Oversee clients' medical / nursing needs and ensure service delivery according to service contract and maintains documentation accordingly
- Collaborate and co-operate with internal and external care team members for co-managed cases
- Conduct education and training sessions for caregivers and / or clients to promote person-centred care needs
- Conduct in-service training sessions to guide and support staff and volunteers
- Develop and review workflows or Standard Operating Procedures (SOP) to aid operation needs
- Recommend new strategies for service development and quality improvement / excellence to supervisor and / or manager
- Be a good team player, building positive influence to team and motivated to learn and grow towards self-excellence
- Perform any other duties as directed by supervisor and / or the management
QUALIFICATIONS
- Diploma/Bachelor Degree in Nursing
OTHER INFORMATION
Relevant Experience
- At least 2 years of relevant experience. Experience with community nursing is preferred
- Fresh graduates are welcome to apply
