Community Outreach Team (CREST), serves as a community safety network for people with or at-risk of mental health conditions, including dementia, and their caregivers. Roles & Responsibilities:

Reporting to the Group Supervisor, the Programme Coordinator/Social Work Associate works closely with the team to serve clients and caregivers.

• Assist to plan, coordinate and execute outreach activities to educate the public, seniors and partner stakeholders on dementia and mental health awareness.
• Assessment and review information and referral service and monitoring of clients with mental health needs, and dementia.
• Networking with community partners.

Candidate Profile:

• Diploma required for Program Coordinators. Diploma in Social Work will be preferred for Social Work Associates.
