Job summary

The Senior/Social Worker is responsible for providing comprehensive support to clients transitioning from Home to independent living. The role involves ensuring a smooth transition and providing ongoing guidance and assistance to empower youths in various aspects of life.

Key Responsibilities:

Case Management

• Collaborate with Case Executives from the residential services to identify clients needing post-care transitional support.
• Assess referrals, coordinate triage with professionals, and determine suitability for aftercare services.
• Develop personalized transition plans with clients, addressing their needs, strengths, and goals.
• Engage with community stakeholders to address systemic barriers to reunification.
• Implement, monitor, and update case plans and interventions.
• Provide ongoing support for independent living, including educational, employment, and life skills support.
• Regularly report client progress to inform case plan decision-making.
• Support reunification efforts for clients and families.

Community Engagement

• Build and maintain a robust network of community partners and governmental agencies.
• Mobilize resources to meet client needs during and beyond the transition period.
• Coordinate accommodation options for clients lacking housing stability.

Administrative

• Ensure timely and accurate documentation and submission of work.
• Consolidate and verify monthly statistical data.

Other Duties

• Attend staff meetings, supervision sessions, and training courses.
• Participate in special projects, programs, and committees within CSLMCH.
• Supervise Social Worker Associates.
• Perform additional duties as required by the position or requested by the Supervisor.

Academic &Trades Qualifications

Essential:

• Bachelor's degree in Social Work or related social sciences (Psychology & Counseling)
• Minimum 2 years of relevant experience in children, youth, or family

Desirable:

• Master’s degree in Social Work or related social sciences (Psychology & Counseling)

• Check all drawings and prepare RFI for all discrepancies

Requirement:

• Candidate must possess at least Diploma/ Degree in Architecture or equivalent

Role Description (Primary)

OVERVIEW

The role of a driver in DAC is to provide safe driving and high standard of safety and care for staff and clients. The driver is also required to assist the DAC’s general office in documents despatch services, logistical transportation and other general works support.

RESPONSIBILITIES

1. Transport Assignments which includes:

a) Co-ordinate with the DAC general office on transportation assignments on a daily/weekly basis and carry out the assignments by driving assigned vehicle(s);

b) Inform the DAC general office immediately if any of the transportation assignment(s) cannot be carried out;

c) Responsible to send the staff/ clients to and from assigned venue/activities safely and according to schedule of the assignment;

a) Keeping vehicle in good and serviceable conditions at all time;

d) Coordinate maintenance, servicing and repair of the vehicles;

e) Obtain quotations for required maintenance, repair & modifications (in compliance to LTA) to the assigned vehicles;

f) Payment of road tax upon receiving road tax renewal notice, cheque and copy of vehicle insurance from the DAC General Office; and

g) Sending vehicles for statutory inspection upon receiving LTA’s Vehicle Inspection Notice.

3. Cover the driving transportation assignment for other programmes/departments (i.e. School/ EIPIC/ HQ whichever is applicable), whenever the need arises.

4. Perform other general works duties as and when assigned by the Management on the days with no driving transportation assignments, e.g. helping in class.

5. Attend and participate in DAC and Association meetings/ activities as and when required.

Who are we looking for?

• Bachelor’s degree in Computer Science, Engineering, or related field, or equivalent experience.
• Proven experience as a Senior Technology DevOps Engineer or in similar IT roles.
• Proven experience in administering and supporting ERP System, HR System, MS Sharepoint and Corporate Website.
• 1-2 years of experience in project management. PMI certified or experience in Agile project methodology is good to have.

COMPANY DESCRIPTION

As a member of Gracehaven Social Services Team, you are expected to provide effective Case Work Services for children, youth and their families. To partner with the different professionals to deliver high quality of care to meet the diverse needs of children and young persons entering the residential service. To ensure that the emotional, social, behavioural, developmental, therapeutic and educational needs of clients are met during the client's stay in the Home. You will need to support the Manager in guiding and ensuring work delivery in the Service and observance of Standard' of Care, Social Work practice and ethos.

RESPONSIBILITIES

1. Case Work Management

• Effective use of therapeutic approaches and other intervention strategies to stabilize and regulate emotions of clients.
• Ensure behaviour support management strategies are in place to effectively manage challenging behaviours of clients.
• Be trauma-informed in the management of cases.
• Be able to work with both young people and their family members.

1. Community Resources and networking

• Establish close working partnership both internally (Gracehaven Children's Home, Gracehaven IPAC and Residential program Team and Volunteers) and other stakeholders so as to maximise community resources and support systems.

1. Programme Development and Evaluation

• Plan and implement programmes that will complement the goals of the Service and that are engaging and therapeutic in intervening with the children and young persons.
• Evaluate the outcome of the project/Program against the objectives on a regular basis.

1. Service Development

• Advocate and represent the rights, needs and welfare of Clients through official platforms, to address service gaps, concerns or issues that will promote and enhance the service delivery for beneficiaries.

1. Professional Development

• Promote professional development through identifying learning needs, transfer of skills and knowledge and offer training where appropriate.

1. Administration

• Complete all necessary documentation for all clients including admission forms, discharge summaries and other ongoing records.

• Degree or Graduate Diploma in Social Work from a recognized university
• Accredited as Registered Social Worker with SASW.
