Key Responsibilities:

• Welcome and engage with patients, creating a warm and inviting atmosphere from the moment they walk through our doors.
• Assist chiropractors with patient assessments
• Provide education and answer basic questions about chiropractic treatment procedures, helping patients feel informed and at ease.
• Schedule appointments, manage patient records, and handle administrative tasks efficiently, ensuring a seamless patient experience.
• Maintain accurate and up-to-date documentation of patient information and care plans.
• Manage insurance claims and billing processes with attention to detail.
• Ensure proper maintenance, organization, and cleanliness of equipment and office spaces.
• Act as a liaison between chiropractors and patients, effectively relaying information and instructions to ensure clear communication.
• Take videos and pictures for our social media platforms, contributing to our online presence and patient engagement.
• Assist in coordinating and supporting corporate talks, wellness bazaars, and other outreach events.
• Supervise and train other assistants or new staff members when necessary, fostering a collaborative and supportive team environment.

Troubleshoot IT Network issues, help desk ticket support, hardware setups and software applications

• Support system upgrades, installations, and software updates
• Assist in the administration of user accounts, permissions, and access rights and set up email accounts
• Maintain accurate and up-to-date IT inventory records, including equipment and software licenses
• Provide IT support to local and remote users (Hardware/ Software/ Network)
• Attend to ad-hoc service repair calls
• Assist in web design and graphic design work
• Any other duties assigned by the Superior

Requirement

1. Diploma or degree in Computer Science, Information Technology

2. Interested in marketing IT tools and web services

3. Good communication skills. People-oriented and able to speak in public/zoom briefing

4. Provide support outside of normal operating hours as required

5. IT Savvy and familiar with IT tools such as Microsoft Office and Microsoft Access, SQL database, Phyton, C++, Java, etc

6. Able to commit to a minimum 6-month to 12-month intern contract.

As Assistant Care Coordinator, Primary Care Partnerships in RHS Office, you are responsible for the following areas:

Job Responsibilities

• Responsible for the coordination of care of residents under the care of the General Practitioners (GPs) and Nurse Practitioner (NP).
• Support GPs on primary care initiatives and/or Primary Care Network (PCN) related matters.
• Data entry into database including tracking and coordination to ensure that patients follow the care plan for preventive and chronic diseases as laid out by the GPs and NP.
• Participate in committees or projects to improve care processes related to preventive care or chronic disease management.
• Promote health screening and wellness to the patients through care planning and coordination of care in the community.
• Provide health coaching to PCN patients and work closely with PCN GPs to improve clinical outcome.
• Coordinate social and community referrals eg. Active ageing centre AACs, IRMS services, etc.
• Provide support in quality improvement projects.
• Perform any other administrative duties as assigned by supervisors.

Job Requirements

• Possess a Diploma (Preferably in Health or Social related)
• At least 1 – 2 years of relevant work experience (Fresh Graduate are welcome to apply)
• Strong communication and personal engagement capabilities of older adult in the community
• Able to multi-task and work independently and as a team
• Strong organisational, coordination, interpersonal and communication skills
• Able to respond to common inquiries or complaints from doctors, staff and patients
• Proficient in Microsoft Office
• Able to converse in local languages and dialects is preferred.

Job Description:

As an Assistant Care Coordinator in CareHub, you will be required to:

· Provide case management via a coordinated process of care planning and coordination, conduct regular phone call follow up and telehealth monitoring under the supervision of a Care Coordinator.

· Work with the Care Coordinators and nurses to develop and manage care for the patients or residents in the community.

· Act as a health coach and provide health screening for the residents in the community and health post.

Requirements:

· Diploma in the following discipline is preferred. (Diploma in Health Management and Promotion, Diploma in Community Development, Diploma in Social Science in Gerontology [GEM])

· Prior relevant work experience will be an advantage. Fresh graduates are welcome to apply

· Good knowledge of various community services and support in the community setting will be an advantage

· Able to work as a team

· Strong organisational, coordination, interpersonal and communication skills

· Proficient in computer skills and Microsoft Office

· Able to converse in local languages is preferred.

· Able to work on weekend (on a rotation basis)

• Engage internal and community stakeholders e.g. grassroots organisations and social service agencies with operations team to reach out to the residents

• Plan & execute engagement events to promote health programs to residents

• Support in planning & implementing Healthier SG engagement initiatives

• Understand the needs of the residents through outreach and feedback to planning and operations team
