We are seeking a dedicated and compassionate Social Worker to join our team. The ideal candidate will be committed to providing casework and counselling to assist clients in smoothly transitioning out of residential homes or to permanent fostering placement. To provide guidance, support and advocacy to empower the clients to regain their independence and successfully integrate back into the community.
Job Duties and Responsibilities
Case Management
· Work with voluntary children’s home case workers to identify clients who require post-care transitional support.
· Assess the suitability of referrals and arrange for triage with professionals involved to determine suitability for enrolment.
· Work with clients to identify needs, strengths and goals, develop personalized transition plan and work with community stakeholders to address systemic barriers to reunification.
· Implement and follow up on case plans and interventions.
· Participate in case conferences and multi-disciplinary meetings to review client’s progress and progression to next phase.
· Provide ongoing support and guidance to empower clients to develop skills for independent living. This includes educational support, supported employment, community integration and life skills training
· Provide regular updates and reports to the interdisciplinary team to facilitate decision-making on case plans for the clients.
· Develop tools and programs to support reunification work for CYPs and families.
Administrative
· Ensure timely and accurate documentation and submission of work
· Consolidating and ensuring accuracy of monthly statistics data
Community Engagement
· Build strong network and collaboration with community partners and governmental agencies to provide coordinated care by mobilizing resources to meet the client’s on-going needs during and beyond the transition period
· Coordinate accommodation options for clients who do not have housing options where required.
Other Duties
· Attend staff meetings and supervision
· Attend internal and external courses and training
· Participate in special projects/programmes and committees in CSLMCH
· Perform additional duties as required by the nature of the position or as requested by the Supervisor
Candidate Requirements
· Degree or Masters in Social Work with a minimum 2 years (or minimum 4 years for Senior Social Worker position) of related work experience in children, youth or family work.
· Degree in social sciences (psychology & counselling) will also be considered
· Demonstrates some understanding of reunification work, trauma and attachment theories, and child and adolescent development.
· A positive attitude with the ability to think out of the box.
· Ability to manage stress and crisis effectively while making calm and informed decisions.
· Receptive to feedback from others with a willingness to improve and grow oneself
· Good communication and interpersonal skills – able to work individually and within a team, relate easily to other people, particularly young person, and form positive working relationships.
· Ability to communicate bilingually is a plus (English and Mother Tongue/Dialects)
· Willing to work after office hours as this is a 24/7 service and flexibility in work arrangement is expected when need arises.
