RESPONSIBILITIES
You will be part of the Community Case Management Service (CCMS) team, providing support to vulnerable seniors and their caregivers who face difficulties in managing their multiple health and social care needs. Key responsibilities:
- To provide case management for CCMS cases
- Review staff's case assessment, documents for submission
- Lead and oversee social work practice development in CCMS
- Provide and set direction for supervision and staff's professional and personal development in agency
- Conduct coaching and performance management for staff
- Contribute to Integrated Case Management Core Team in service planning and development, implementation of social-health integration and practice development.
- Networking and collaboration with stakeholders to provide intervention support to client and/or develop resources or strategic partnership
- Represent agency in regional and sector platforms for networking, collaboration and partnerships (SSAs, RHS)
- Support other CCSS programmes as required.
Degree/Graduate Diploma in Social Work
Requirements:
- At least 3 years of relevant experience
- High proficiency in written and spoken English; able to speak in mandarin and/or dialects would be essential for engagement with seniors
- Passionate to work with seniors and caregivers
- Strong interpersonal and communication abilities with individuals and systems at multi-levels
- Good analytical and assessment skills
- Team player with an ability to function effectively in a fast-paced environment
- Strong organisational skills and structured.
