The Community Nurse (Home Care) is responsible for planning; coordinating, directing and evaluating the patient care services and provides supervision serving as the vital link between the vision of CWA and a team of Community Caregivers (CCGs). It served as the unit-based delivery of effective, cost-efficient high quality patient care. It also includes providing quality care to clients under the Home Monitoring System.
The Community Nurse (Home Care) is also responsible for managing and assist in providing direct patient care through care management, assessment, providing home personal care services, nursing procedures and evaluation of nursing care to meet the needs of the seniors, including educating patients & families and caregiver training.
The Community Nurse (Home Care) needs to assist the Registered Nurse in the delivery of required and optimal level of healthcare services to clients at home.
DUTIES AND RESPONSIBILITIES
1) Home Personal Care (HPC)
a) Patient Care
· Assess, plan, implement and evaluate patients’ needs
· Provide management, scheduling and supervision a team of Community Caregivers (CCGs) and in-house training of CCGs
· Development, implementation and review of clients’ home care plans
· Facilitate patient flow and patient education
· Refer patients for further care or services, when necessary
· Serve as a clinical resource person for CWA and CCGs
· Provide home personal care services (similarly to the CCGs) if needed
· Provide direct home nursing care and procedures
· Contributes to patients’ assessments and care plan reviews under the supervision of a Registered Nurse (RN)
· Conduct home and hospital visits
· Ensure level of standard in patient care
· Ensure quality standards and up-to-date healthcare trend
2) Digital Health/Care Programme
· Work with management to roll out new health/care programmes for clients
· Content and process expert if customization needed.
· Provide training to seniors/CCGs
· Ensure programme/systems are rolled out successfully
3) Administration and Reports
· Plan and co-ordinate the recruitment, and retention of CCGs, inclusive of CCGs engagement programmes
· Development and co-ordination of training for CCGs
· Planning of CCGs roster, deployment of CCGs, as well as provide in-service training for CCGs, whenever necessary
· Ensure smooth daily operation flow
· Development and review/revise of Home Personal Care and CCGs Standard of Operations (SOPs) manual when necessary
· Handle administration work and reports:
- Manage and maintain clients/cases database and communications.
- Supervise the maintenance of CCG records, time sheets, medical treatments, SBAR and related services
- Handling of client’s Service Contract and collection of payment
- Handling of CCGs’ Contract and mass briefing / interview
- To prepare weekly/monthly activity and statistic reports
- Preparation and submission of patients’ subvention to MOH/AIC
- HPC, Outreach Programme reports and statistics, including Home Monitor System
- Conducting of surveys to ensure service quality, when necessary
· Provide administrative and support to other staff members in programmes and coverage.
4) Other Duties
· Assist and support all ad-hoc events organised by CWA and/or third party organisers when assigned, and assist in any other events of the CWA which may be held in the evenings and possibly on Saturdays, Sundays and Public Holidays.

Requirement criteria:
- Candidate must possess NITEC in Nursing.
- SNB Registered
- At least 1 year of working experience in the related field is an advantage
- Demonstrate good communications and people engagement skills
- Good working attitude and ability to work in a team
- Good communication and interpersonal skills
