Reporting to the Resident Physician, this role functions primarily as the case manager for SingHealth Community Hospitals (SCH) inpatients referred into SCH’s post-discharge programmes (e.g. Integrated Primary Care for At-Risk Elders [iPCARE]). As these post-discharge community programmes are refined over time to enhance care models according to the operating environment, the case manager, as a key care team member, will participate actively in the evaluation of programmes and related quality improvement projects.
