You are responsible for assessing, developing, implementing, and evaluating the clinical practice setting and standards of care for the Community Psychogeriatric Programme.
Responsibilities:
• Coordinate patient care with the Community Psychogeriatric Programme (CPGP) team
• Provide on-site home-based assessment, including mental assessment and implement nursing care
• Assess, Plan, implement and evaluate nursing care plan
• Deliver prescribed treatment and procedures
• Provide accurate and timely documentation of patient’s progress, nursing care and treatment
• Maintain effective communication with patient’s family or caregiver and provide education and support according to their needs
• Conduct training, consultation and support to staff and volunteers of community eldercare agencies to increase their knowledge in caring for the elderly with mental disorders
• Assist in managing and maintaining the CPGP database, and managing email/phone enquiries related to CPGP
• Conduct on-site mental health elderly screening via referrals from community partners
Requirements:
• Diploma / Degree / Advanced Diploma in Nursing
• Registered Nurse with Singapore Nursing Board
• Experience in Psychiatric or Gerontology Nursing would be advantageous
