Key Responsibilities:
1. Resident Care:
· Assess residents’ health conditions and develop personalized care plans.
· Administer medications and treatments as prescribed by physicians.
· Monitor and record residents' vital signs and medical information.
· Provide wound care and other nursing procedures as required.
· Assist residents with activities of daily living (ADLs), including feeding, bathing, and mobility.
· Respond to medical emergencies promptly and effectively.
2. Supervision and Mentorship:
· Supervise junior nursing staff, nurse aides and healthcare assistants
· Provide guidance and mentorship to junior staff, fostering a collaborative and supportive work environment.
· Conduct regular training sessions and ensure staff are updated with current nursing practices.
3. Communication and Coordination:
· Communicate effectively with residents, families, and healthcare professionals.
· Collaborate with multi-disciplinary teams to ensure comprehensive care for residents.
· Participate in care plan meetings and provide input on residents’ care and progress.
4. Quality Improvement:
· Participate in quality improvement initiatives and contribute to the development of best practices.
· Ensure compliance with regulatory requirements and standards of care.
· Conduct audits and participate in continuous education programs.
5. Documentation:
· Maintain accurate and up-to-date medical records and documentation in the Healthcare System.
· Ensure all documentation meets legal and organizational standards.
· Report any changes in residents’ conditions to the appropriate healthcare professionals promptly.
6. Safety and Infection Control:
· Ensure a safe and clean environment for residents and staff.
· Adhere to infection control policies and procedures.
· Promote and practice proper hand hygiene and other safety protocols.
Qualifications:
· Registered Nurse with the Singapore Nursing Board.
· NITEC or Diploma or Degree in Nursing from a recognized institution.
· Advanced Diploma in Gerontology will be an added advantage
· Minimum of 2-3 years of nursing experience, preferably in a geriatric or long-term care setting.
· BCLS and AED certification.
· Strong clinical skills and knowledge of geriatric care.
· Excellent communication and interpersonal skills.
· Ability to work independently and as part of a team.
· Leadership and supervisory experience (for Senior Staff Nurse position).
