RESPONSIBILITIES

The KidSTART Practitioners role is to provide targeted assistance to children and families enrolled in the KidSTART programme. Key responsibilties include:

A. Intake and Referral

• Process referrals from a variety of sources, including hospitals, community referrals, self-referrals, government agencies and other Social Service Agencies (SSAs)
• Engage families about the programme, seek consent from and determine eligibility of families for KidSTART programme.
• Collate and maintain accurate and complete set of data in the Social Service Net (SSNet) and on internal databases for all referrals and intakes.

B. Needs Identification and Assessment

• Conduct regular screening and assessment using different tools and assessments to ensure childs developmental needs are met and support maternal/main carers well-being.
• Develop holistic child-centric case plans to coordinate required support for the identified children.

C. Case Coordination, Systems Linkages and Mobilisation

• Work closely with community partners/stakeholders to enhance service delivery and promote positive child development outcomes.

• Tap on a team of multi-disciplinary professionals to support health and developmental needs of identified children.

D. Education & Equipping
