The Community Nurse (Home Care) is responsible for planning; coordinating, directing and evaluating the patient care services and provides supervision serving as the vital link between the vision of CWA and a team of Community Caregivers (CCGs). It served as the unit-based delivery of effective, cost-efficient high quality patient care. It also includes providing quality care to clients under the Home Monitoring System.

DUTIES AND RESPONSIBILITIES

1) Home Personal Care (HPC)

a) Patient Care

· Assess, plan, implement and evaluate patients’ needs

· Provide management, scheduling and supervision a team of Community Caregivers (CCGs) and in-house training of CCGs

· Development, implementation and review of clients’ home care plans

· Facilitate patient flow and patient education

· Refer patients for further care or services, when necessary

2) Digital Health/Care Programme

3) Administration and Reports

· Plan and co-ordinate the recruitment, and retention of CCGs, inclusive of CCGs engagement programmes

· Development and co-ordination of training for CCGs

· Planning of CCGs roster, deployment of CCGs, as well as provide in-service training for CCGs, whenever necessary

· Ensure smooth daily operation flow

· Development and review/revise of Home Personal Care and CCGs Standard of Operations (SOPs) manual when necessary

· Handle administration work and reports:

- Manage and maintain clients/cases database and communications.

- Supervise the maintenance of CCG records, time sheets, medical treatments, SBAR and related services

4) Other Duties

· Assist and support all ad-hoc events organised by CWA and/or third party organisers when assigned, and assist in any other events of the CWA which may be held in the evenings and possibly on Saturdays, Sundays and Public Holidays.

Veterinary Assistants

Awesome if you have some experience, you will be assisting our veterinary technicians and receptionist in their duties that will involve both the front desk and hospital work. We are open to candidates with relevant veterinary skills who are returning to the workforce. Confident animal handling skills and willingness to learn is a must!

How will you grow with Gaia Veterinary Centre?

You can expect lots of opportunities to experience the whole spectrum of veterinary medicine that include (but not limited to) orthopaedics, physiotherapy, advanced imaging, in-house diagnostics and even exotic animal medicine.

• Perform reception duties, registration, payment collection and administrative duties relating to patients.
• Provide assistance to the Manger in ensuring the smooth operation
• Perform triage and assist during Emergencies.
• Render quality nursing care to patients according to the patient according to their needs.
• Assist doctors to carry out clinical/medical procedure such as venepunctures, dressings, suturing, removal of stiches and etc.
• Observe strict infection control practices in the care of patients.

Job Requirements

• Enrolled with the Singapore Nursing Board
• Good interpersonal skills with a pleasant personality
• Able to work in a dynamic environment as well as being flexible and open to changes

• Perform patient assessment and clinical intervention in hospital.
• Provide quality nursing care for an assigned group of patients.
• Perform nursing procedures and treatment to meet the general comfort and well-being of the patients.
• Competencies in clinical nursing skills and knowledge such as performing IV cannulation, venipuncture for blood sampling, IV administration of medical fluids, ECG monitoring and interpretation are preferred.
• Assist doctors and attend to emergency calls when necessary
• Provide patient education and participate in patient rehabilitation, involving the patients’ family as necessary.
• Record and maintain proper accurate documentation of patient's progress and management rendered in the notes, medication records and charts.

Job Requirements

• Candidate must possess at least a Degree in Nursing
• Registered nurse with the Singapore Nursing Board
• Preferably 5 years of inpatient/ward working experience
• Able to work in a dynamic and fast paced environment
• Good communication and interpersonal skills

Job Description

• Minimum 3 years’ experience will be advantageous.
• 1) Assess and identify the individual needs of clients by using specific assessment tools.
• 2) Formulate, develop and implement appropriate rehabilitative programs.
• 3) Monitor and evaluate clients' individualize needs and progress.
• 4) Be part of the multi-disciplinary team of various disciplines in the program.
  5) Educate and Train caregivers in carrying out home therapy and program.
  6) Keep abreast of current research and trends and assist in data collection and support action research activities.

• 1) Recognised Diploma / Bachelor Degree in Occupational Therapy.
  2) Registered or eligible for registration with the Allied Health Professional Council in Singapore as Full Registrant.
