Resident Management

RESPONSIBILITIES

• Provide Home Based Nursing Services
• Conduct Caregiver training
• Perform biopsychosocial and environmental assessments to identify strengths and needs of clients, families and/or caregivers.
• Plan, develop and evaluate person-centred nursing care plans with considerations of client's needs and preference. Communicate care plans and address the concerns from patient and family.
• Implement and evaluate care interventions(s) e.g. wound care, medication management, continence care,; ensuring at all times:
• Practice safety and infection control precautionary measures.
• Manage patient workload through timely planning and scheduling based on the client's needs. Utilise workforce optimiser to actualise the planned visits.
• Build therapeutic relationships and empower the clients, families and/or caregivers for self- management through health promotion, caregivers training and motivational techniques.
• Coordinate with multidisciplinary team for care integration and transition
• Initiate emergency protocols when necessary.
• Discuss / escalate to Supervisor and/or Care team on any problems encountered in patient's condition and/or in the handling of patients and/or care-givers/family members.
• Develop discharge plan and refer patients to other service provider to transit care.
• Supervise the work of the Enrolled Nurses in his/her zone, as well as nursing students on clinical attachment to achieve their learning objectives.
• Perform visits during Emergency Nursing Services for normal and extended hours.
• Maintain timely and accurate documentation of patients' condition and care given.
• Collect correct fees by checking the subsidized status of patients, issue receipt and submit the collection in a timely and accurate manner.
• Facilitate application of healthcare financial scheme such as Senior Mobility Funds when appropriate.
• Comply with guidelines and policies to ensure client confidentiality and personal data protection.
• Keep abreast of new technology and nursing knowledge through reflection on practice and continuous professional development.
• Facilitate Advance Care Planning (ACP) discussions with client and/or family and/or caregiver and ensure proper documentation.
• Facilitate grief and bereavement support and after care to the bereaved family and / or caregiver and staff.
• Support phone calls in rotating shifts duty including weekends and Public Holidays.

• Assist the (Senior) Nurse Manager and/or Nurse Clinician to improve, facilitate and implement changes within the home nursing work processes.

• Registered Nurse with the Singapore Nursing Board
• Degree in Nursing and/or Advanced Diploma in Palliative Care or other specialty preferred.
• Minimum 3 years' experience working in working in acute hospital setting and/or community hospital setting.
• Possess good communication skills to establish therapeutic relationship that fosters optimal quality patient care.
• Able to make clinical judgements regarding patient care with minimal guidance.

Brief Description

Reports directly to the Executive Director, and will be responsible for the full spectrum of finance and accounting matters in BCARE. One of the key requirements in this role is the need to ensure accuracy, completeness, and compliance with internal controls for all financial transactions. I Annual Budgeting

1. Work closely with the various Centre Heads to develop the annual budget and seek approval from the Management Committee.

2. Review ad-hoc budget requests, ensuring compliance with the organisation's fiscal policy.

3. Review budget variances at the end of the financial year (actual vs budget for key centres and programmes e.g. SAC, JOYC, FAME Club).

II Funding and Investment Matters

4. Handle funding matters related to MSF/ NCSS/ MOH/ AIC and BCARE’s funders.

5. Support and provide timely updates on funding/ project/ fundraising.

6. Handle Fixed Deposits and investment matters.

III Annual Audit and AGM

7. Work closely with various stakeholders including auditor, staff to close the accounts for annual audit.

b) maintain internal controls related to financial expenditures, procurement and payment/claims such that risks are mitigated (e.g. document and maintain complete and accurate supporting information for all financial transactions).

c) maintain all financial accounting systems and controls.

d) in charge of Asset Management Policy, implementation and execution e.g. tagging of assets and record-keeping.

19. Day to Day Accounting includes:

a) book-keeping and monthly bank reconciliation including maintenance of proper accounting records for the general ledger, accounts payable, accounts receivable, and payroll.

b) track and manage the different types of restricted funds/ funding schemes for BCARE and its satellite centres.

c) handle all daily cash management including managing money that comes into, within cash receipts and payments).

d) handle all accounts payable including checking and processing invoices, payments, petty cash, expenses, claim and reimbursement following internal controls/ procedures.

e) handle all accounts receivable; manage BCARE’s credit standing to ensure that all invoices are billed and collected timely.

f) take charge of all the necessary VCF/SDF claims for the Organisation.

g) safeguard other finance-related instruments/ contracts including securities, and other valuable papers (cheques, subscriptions, tenancy agreements/ stamp fees etc.).
