Building on more than five decades of experience and expertise, NTUC Health is among the largest senior day care, nursing home, and home personal care providers in Singapore. We also offer other services for seniors such as active ageing centres, community support for vulnerable seniors, and a sheltered senior group home. In addition, we run a chain of dental clinics and a family medicine clinic.

RESPONSIBILITIES

In this role, you would be responsible for leading and managing frontline operations of the Home Care department, particularly in the area of optimising manpower and provision of quality customer service and care. You would be working closely with internal stakeholders in the execution and revision of operations policies.

To foster the integration of social and health care needs of the client, you would be required to engage relevant external stakeholders such as social and health partners to develop pilots, and process mapping of client journeys.

Service Coordination

• Conduct and participate in outreach programme to generate awareness of the service and to acquire new clients
• Coordinate services requested by new clients. Maintain proper documentation in the systems and ensure hard copy filings are completed in a timely manner
• Conduct regular home visits to build rapport and engage clients and their caregivers
• Follow up with clients via regular phone calls to engage and gather feedback on our services
• Collaborate with other social and health partners to recommend care programs according to clients needs.
• Monitor Module E Client Onboarding technology during office hours
• Counsel and educate caregivers on providing appropriate care and support to clients at home.
• Arrange for a reassessment of care plans with clients on a 6-monthly basis to determine if the client requires continuous service
• Work closely with the Operations and Nurse Manager on discharging clients from the service

Manpower Deployment for Frontline Operations

• Responsible for the recruitment of care staff and development of a robust onboarding training programme
• Provide supervision to care staff by providing timely and regular supervision, coaching on their service delivery to clients
• Work closely with nursing team on their skills competency to ensure the service quality outcome rendered to clients is met
• Support home personal care services and assist them with clients daily activities of living on an ad hoc basis

Quality Care

• Establish and evaluate processes and policies to ensure the smooth delivery of services and in accordance to MOH guidelines
• Ensure client satisfaction delivery through constant evaluations of areas of gaps to implement changes to ensure client satisfaction is maintained at 90%
• Participate in family conference, and case discussions on cases with complex needs
• Prepare incident reports and review to continuously improve on gaps that can be mitigated

Community Partnership and Engagement

• Engage and established local partners from social and healthcare sectors to support NTUC Health clients
• Develop and pilot workflows to support ground-up initiatives

Communications

• Coordinate and communicate with internal support functions such as IT, HR and Finance, and external stakeholders to ensure smooth operations of Home Care services
• Deliver consistently good customer service including inquiries, and complaints at all times
• Handle phone and email enquiries of current and prospective customers of NTUC Health
• Advise and promote the value of our NTUC Health services to the customers

Key Performance Indicators (KPIs)

• Conduct regular outreach on a fortnightly basis to connect and reach out to the residents
• Enrollment and retention of 60 clients per site
• Ensure staff utilization to be maintained at 90%

1. Over-all in-charge for Planning and Site Operation

2. Ensures all company’s ISO, safety, environment policy and procedures are complied with.

7. Closely monitoring of site progress and to resolve whatever site obstruction to expedite works.

8. Monitor Coordinators & Engineers and ensure all communication will transmit to all parties (Architectural, Structural & M&E – staff & subcontractors)

9. Monitor and chase Contract Department to award subcontractor works

10. Monitor and chase Materials sample submission for all trades to consultant / client approval.

11. Monitor and assist Coordinator & Engineer to Prepare Method Statement for approval.

12. To assist Coordinator and Engineer for preparation of required Mock Up unit.

13. Ensure that all information is distributed to all site staff, sub-contractors, suppliers and all other trades involve in the project.

14. Correspondences to Consultants, Subcontracts and HQ.

15. Maintains and ensure all testing requirements and records are in place.

16. Ensures all discrepancies are resolved with the consultants in the shortest time required.

17. Monitor daily activity report from respective site staff

18. Liaise with, Structure Engineer, Architectural Coordinator and M&E Engineer for daily progress of work to ensure meet the target schedule of the Project completion.

19. Performs daily site inspection to ensure work progress is align to the schedule.

20. Performs daily inspection to ensure safety provision on site.

21. To ensure all safety documents are in place to get ready for any Authority Inspection.

22. Performs daily site inspection to resolve site problems with the structure Engineer, Architectural Coordinator, M&E Engineer & Site Supervisors.

23. Accompanies Consultants and Client for any site inspection if neccesary

24. Monitor all site works to ensure they follow according to contract and specifications.

25. Conducts site meetings as and when required in regard to site issues.

26. Inspect works carried out by subcontractors are in-accord with approved trade demonstration

· Candidate must possess at least Diploma in Finance/Accountancy/Banking or equivalent.

· At least 1 Year(s) of working experience in the related field is required for this position.

The Role

The Healthcare Assistant will provide support and assistance to the Nursing staff in providing the residents with basic hygiene and personal care daily, attending to their needs and notifying attending nurses of any changes in the physical or emotional state of residents.
